Liability Waiver & Release
RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND INDEMNITY AGREEMENT
ALPHA BEAR CLUB — ARCHERY RANGE & ADVENTURE EQUIPMENT
By signing this document, I acknowledge and agree to the following:
1. ASSUMPTION OF RISK: I voluntarily participate in archery shooting, adventure training, and related activities at Alpha Bear Club. I understand these activities involve inherent risks including, but not limited to: injury from arrows, bows, firearms, tools, or equipment; physical strain; and potential accidents. I accept all risks associated with my participation.
2. WAIVER OF LIABILITY: I hereby release, waive, and discharge Alpha Bear Club, its owners, employees, coaches, and affiliates from any and all liability, claims, demands, or causes of action arising from my participation in activities, including any injury, damage, or loss.
3. MINOR PARTICIPATION (for participants under 18): I, the parent/legal guardian, give permission for the minors named below to participate in activities at Alpha Bear Club. I agree to the above terms on behalf of each minor and accept all responsibility for them.
4. HEALTH & SAFETY: I confirm that I (and each minor) am physically capable of participating and have no medical conditions that would prevent safe participation. I agree to follow all safety rules and instructions from Alpha Bear Club staff.
5. GOVERNING LAW: This agreement shall be governed by the laws of the State of Michigan.
I HAVE READ THIS RELEASE OF LIABILITY AND FULLY UNDERSTAND ITS TERMS. I SIGN IT FREELY AND VOLUNTARILY.
